There were multiple factors that contributed to the death of a resident of the Parkside Manor nursing home in Kenosha, Wisconsin, according to a state report that was just released.
The resident was admitted to the facility in June 2022 after being diagnosed with dementia. In December, she walked out of the facility late at night, which caused an alarm to go off. A staff member thought that another resident had caused this to happen and did not investigate any further.
The state report says two staff members tried to silence the alarm but did not open the door enough to be able to see outside. Staff members discovered the resident with dementia the next morning just 20 feet away from the door. She died due to exposure to the weather.
The state report notes staff members failed to follow procedures that required them to frequently check on the resident. Procedures also called for an immediate canvassing of the facility as a response to the alarm that went off.
According to the report, one of the caregivers who was scheduled to be there that night was late and was not present for part of the shift.
Wisconsin’s Department of Health Services proposed that the facility stop admitting residents until changes are made to facility procedures and staff members are trained. DHS also proposed a fine of $1,900.
There is still an open investigation by police to determine if there should be criminal charges.
