Nursing Home Staff Sexual Assault Case: Correctionville Facility Under Investigation

In October 2023, a female resident at Correctionville Specialty Care reported that a male staff member had sexually assaulted her, alleging he raped her in his car and sent explicit videos. She provided authorities with video evidence of the misconduct. Shockingly, instead of supporting the victim, the facility retaliated by evicting her and dropping her off at a homeless shelter after she reported the assault to authorities.

While Correctionville terminated the accused worker, state inspectors discovered that the parent company, Care Innovations of West Des Moines, allegedly continued employing him at other Iowa nursing homes. Although no criminal charges were filed in this specific case despite video evidence, the same worker was later convicted of sexual exploitation at a different Iowa facility, suggesting a pattern of inappropriate behavior toward vulnerable residents across multiple nursing homes.

State inspectors documented the facility’s retaliation against the victim and cited Correctionville for violations related to resident abuse and care issues. However, despite the serious nature of the sexual assault allegations and the facility’s shocking treatment of the victim, the nursing home faced no fines or financial penalties from regulators.

Severe Staffing Shortages Lead to Neglect

Correctionville Specialty Care has faced 93 state and federal regulatory violations since January 2022, including serious incidents of resident abuse and chronic staffing shortages.

A February 2025 inspection revealed severe understaffing that prevents the delivery of basic care. The facility assigns only two nurse aides to care for 32 residents during overnight shifts, resulting in unanswered call lights and missed baths. Most troubling, staff members admitted to falsifying medical records by writing that residents “refused” baths when they were never offered one, allegedly under the direction of the former nursing director. Despite these serious violations, state regulators imposed no fines or penalties on the facility.

Correctionville Added to Federal Special-Focus List

Since the October 2023 incident, Correctionville has been added to the federal list of Special-Focus Facilities. The federal Special-Focus Facilities program limits each state to no more than two nursing homes at a time. However, hundreds of other facilities nationwide are eligible for this designation due to quality-of-care violations. Only the officially designated facilities receive additional government oversight and assistance to improve resident care, while eligible facilities remain on a waiting list without federal help.

Iowa currently has two designated Special-Focus Facilities: Arbor Court in Mount Pleasant (in the program for over two years) and Aspire of Gowrie (in the program for approximately one and a half years). Since neither facility has achieved graduation through improved care, the only changes have occurred among the 10 Iowa facilities eligible for Special-Focus status.
